Post-Surgical Rehab Made Easy with Ontario’s Bundled Care Program

If you have had a unilateral hip, knee, or shoulder total joint replacement at a participating hospital, you are likely eligible for Bundled Care physiotherapy.


At Delta Physiotherapy and Rehab, we specialize in helping you recover quickly and confidently after joint replacement surgery. As a proud partner in Ontario’s Bundled Care Program, we provide fully funded physiotherapy for eligible patients following:

  • Unilateral Total Knee Replacement
  • Unilateral Total Hip Replacement
  • Unilateral Shoulder Replacement (Anterior or Reverse).


What Is Bundled Care?


The Bundled Care Program is an Ontario initiative that ensures your rehab journey is coordinated and cost-free, from hospital discharge to full recovery. After your surgery, you’ll be referred directly to our clinic for physiotherapy, with no need for private insurance or out-of-pocket fees.

Who Is Eligible?


You may qualify for Bundled Care Physiotherapy if:

  • You had a primary (first-time) joint replacement of the hip, knee, or shoulder
  • The surgery was unilateral (on one side only)
  • The surgery was performed at a participating hospital in Ontario.


Who Is Not Eligible?


  • Patients who had a revision surgery (repeat or corrective joint replacement)
  • Patients who had bilateral surgery (both knees or hips done at once)
  • Surgeries done outside Ontario or at a non-participating hospital
  • Surgery done outside Ontario or at non-participating hospitals
  • WSIB claims (workplace injuries)
  • Major complications during surgery (eg, Stroke or heart attack).

How to Prepare for Knee/Hip/Shoulder Surgery:


If you’re preparing for a hip, knee, or shoulder replacement, your hospital's case coordinator will provide you with a care package upon discharge. You are expected to contact our clinic and schedule an appointment for rehabilitation within two weeks of your discharge. Please bring your eligibility paperwork when you enroll at Delta Physiotherapy & Rehab. We will bill your rehabilitation services directly to the referring hospital.
